Wrong people hired and promoted. A culture where criticism and negative feedback is seen as bad. Likely little testing done. Probably people not payed enough to give a shit. Too many studios/people working on a single game. There's probably a lot of reasons that individually wouldn't have mattered too much, but put them together and you have the Megazord of barely functioning, bland shit.

maybe just maybe, these recent market failures with Dragon Age being the next inevitable candidate will force the industry to focus more on the games and less on the inclusive message in the future
