What caused the underground fire that they was always talking about ?
And i the end, how come they where in another dimension or what you would call it ?

It was when they saved the girl, the coals fell and burned the place. (I think, someone correct me if I'm wrong).

I thought that at first, but then later she is in the silent hill hospital, so surely that means the town was fine (it said that during the fire everyone was evacuated.. so if it started then, she wouldn't have been in the SH hospital..)

The underground coal fire was either started when the coals the girl was being roasted on were spilt everywhere or occured because of an unrelated accident, I think involving a waste fire near an open coal vein.
The idea of underground coal mine fires burning for decades is actually based on a real event. Centralia, Pennsylvania, has underground fires that have been burning for 40 years and could burn for centuries. Most of the population were evacuated but some opted to stay. It's probably an extremely spooky place. I can't imagine the inhabitants are very healthy eihter due to the massive amount of carbon monoxide in the air.
As for them still being in the manifested dimension aka dream world at the end, well I believe that because rose died at some point during the film. Maybe when they crashed, maybe when she was stabbed or whatever. So she was stuck in the 'purgatory' of the dream world Alyssa manifested. The girl wasn't real anyway so she's irrelevent.

never played the games, first time watching the film (few hours ago) the way i figure it is thus:
alyssa is concidered a witch, for whatever reason (it sounded like because her mother would not reveal who the father was?) - anyway, the towns religious extremists (cult, whatever, religion is all the same thing) took her, and burned her. but during the event, the coals she was being roasted over, were spilled, starting a massive fire in the hotel, which killed all the religious nuts, sending them to purgatory... alyssa didn't die (i think?), she was rescued, by the cop who ended up talking to roses husband (he even showed him the scars on his hand from when he tried to unshackle alyssa)
i go hazy here, why was she left behind in the towns hospital when everyone else left? was she rescued, THEN died? her soul or whatever, staying behind in purgatory as well? makes sense i spose...
anyway, alyssa became twisted and bitter at what had happened too her... then the evil whatever came along (equivilent of the devil or some demon? we'll just call it darkness) to help alyssa's torment find an outlet, and get revenge on the bad people in the town... apparently the 'goodness' still within alyssa was seperated and became sharon, which was adopted by rose.
going back to why alyssa is still in the town, when everyone else had left, perhaps when alyssa and the darkness joined, did her hatred physically started to change the world and drove everyone out? OR perhaps when they joined, alyssa 'shifted' into purgatory, and just kinda disappeared from the real world? but then why was the 'red nurse' there? was she pulled across too for some reason? perhaps that bits a plot hole...
silent hill: theres like the outside real world version which her husband visits, theres the middle world version thats covered in mist, and the dark underworld one thats all corrupted, the last 2 stages representing alyssa's state of mind? question: what causes the changes between the two? is it like day and night, where they happen at set periods, or do things happen to cause the change? if so, what are they?
pyramid head shows up during the corruption period, possibly a manifestation of alyssa's anger... a kinda physical enforcer, cutting down those who did her harm, sending them to hell.
the darkness inside alyssa cannot get into the church to send those souls to hell, because their blind belief is so powerful, that it blocks the darkness from getting inside. she needs rose to help her gain entrance. why rose? probably simply because she was the woman who adopted sharon... it could have been anyone, but only someone with that mother/daughter bond would have a strong enough will to do what was required? also because she will see the people for who they are, and what they will do to sharon, so she will be willing to see they get what they deserve... thats atleast how i saw it
the ending: confusing, dont really get it. is rose dead, quite likely... the accident was purposely caused by the darkness, possibly to kill rose, because the only way she could enter that realm was if she was not bound to her body... it explains why they found her car later, but she was gone, they would have towed away her car, but later rose found her car where she left it... doubtful it was a plot hole, as she saw the police bike as well... i doubt the darkness controls entry to purgatory to living beings, as you would opening and closing a door... but why is rose still stuck there afterwards?
something obviously changes in sharon at the end... dont know what, but she definately changes.. guess we need to wait for the sequel on that one...
why was the red nurse crying? what was her relivance and why was she in the room with alyssa in the present? was she caring for alyssa, and thats why she was there? i spose the eye thing was because of her curiosity when she peeked at alyssa... but why keep her, why is she there.
but everything in silent hill, aside from the human characters, is a representation of different mental aspects of alyssas mind...
*first thing u saw that spat out dark acid stuff at the cop: alyssa's feeling of being trapped?
*evil little crispy baby things? literal translations of how she was? or all the kids that tormented alyssa?
*birds... someone explain the birds! originally i thought about the bird in the cage, like canarys were used when the gas was in the air so that miners would know to get out... is this what the birds were used for, and werent actually meant to represent anything?
barbed wire: representation of...? accusations perhaps?
nurses: beauty alyssa will never have? hatred of how the nurses might have treated her in hospital?
creepy dude in the bathroom: colin, janitor... you see him in the flashback, most likely raped her.
its a great film, one definately worth discussing and debating.

This is the point I was trying to make.. I don't think there was a fire at all and I don't think the cult members where killed. How could one building catching fire cause an underground fire? Why was the town perfectly intact?
The hotel caught on fire during the ritual, everyone escaped and continued life while the girl was taken to hospital thought to be close to death. It was while in this hospital all of the pain and anger turned to hate, at which point the "darkness" turns up and tells the girl she can (quote) "pull them all into her worst nightmare" .. This is what caused people to flee the town, and is why they are so protective over it (the cop who gets rid of the dad and who saved the girl..).. The good people escaped the town turning, the bad people where caught in the other dimension of the town to be punished for what they done.
For example, the school janitor who quite blatantly molested her.. he was not a cultist yet he was kept in the world and punished because of the molestation.. He could not have died in the fire, as he was not in the building.
There is a lot to be said about the concept that people have to die to be taken to the world, but for me it doesn't sit right. I'm pretty sure the demon doesn't want the world it is controlling to end so uses rose and Sharon to continue the world by inhabiting Sharon at the end of the movie. This is why they are both still trapped, because the demon has not let her leave.
There will be a sequel.. that is a certainty..
