Well the game engine is a bit old by now, some new shader stuff though from what I can see but it doesn't look all that different from the original game and it's expansion.
(Followed by the stand-alone Overdose version and now this version.)
EDIT: With some new weird weapons and creatures.

So fun to see some ppl who complain and whine about insignificant details on much more complex games, deposit here their love for complete brainless (with a B) shooters.

FUCK this game is a fucking joke, I know painkiller has always been a mindless shooter but AT LEAST make an effort to make new weapons in the game, this game is nothing but recycled everything from painkiller 1.

Tried it. Didn't go well..
