I need to create a survey that can be filled out electronically and then saved, and returned containining the answers. I was initially going to do an html web-based survey, but that isn't possible anymore. This is to collect data for my thesis project.

Can I do this with adobe acrobat? What addons for acrobat will I need? And what versions (ie 5, 6, 7) would this be compatible with? I need more than one option, because it has to be compatible with everyone's computer/operating system/installed applications, with no exceptions.

So far I'm thinking that I'll have a pdf, pdf form, word document, wordperfect document ... am I missing anything?

i don't really get what you want to do. you mean like an offline survey?

if not, why no html? backed up by php and maybe an mysql table to store the answers.

Yes you can use that with Acrobat. It's one of the more popular features of the app. I also know that Microsoft Word 2003 should be able to handle forms.

I don't know enough about it myself to do anything that complicated. It also has to be 100% glitch free on all browsers, which I couldn't guarantee either.

Where I work we have a systems engineer that does this kind of thing. The most I have to do with sql is pull data off the database. I have no money to pay anyone, so I have to do all the work myself.

Any idea how far back acrobat supports forms? And I don't need any addons (I have acrobat professional 6).
