It would be cool if they add "insane" mod to the game, where all humans look like warped version of them-self, and main character considers them alien menace, demons, evil of the world and what not...

That would actually be a cool story, where you play as the protagonist through the whole game and everyone you kill looks like a demon or alien or of the sorts. And when you get a bit in to the game it turns out that the protagonist is actually insane and all this time you've been killing people and not aliens/zombies/demons. Would be a cool twist. ^_^ |

And then the protagonists wakes up and it turns out you actually play as a SJW in a coma, and you actually kill GamerGate people and white cis men, therefore the game gets awarded 20/10 on Kotaku, RPS, Polygon and Giant Bomb and a new wave of articles arrive titled "Hatred brought back my hope in gaming".

The worst thing is that the game looks really cool and graphical style is fantastic
But I really hope the character have some kind of motive and dies at the end

I guess Epic wants to distance themselves a bit but still allow the use of UE4 without creative censoring or what it would be called, as for the police yeah changing the name is easily done and won't impact the game in any way.
(NYPO for example for New York Police Organization instead of Department, or do as in GTA and make the city name fictional too.)
EDIT: That leaves the issue with Steam or GOG or whatever other digital platforms they want it released on, pretty sure no seller currently allows AO rated content so that could be a issue.
(Well I guess Gabe could go crazy and make a AO only version of the client with a separate store, SteamAO?)

Yeah I'm not really sure what their policy is, so far it seems to be rated on a per-game basis with some games being allowed entirely and others getting a restricted version.
EDIT: Sunrider for example - http://steamcommunity.com/app/313730/discussions/0/616189742935652097/ - or that other one that has a Steam version and a Steaming version.

EDIT: I should probably use something more relevant to this particular game and theme of violence, which would be Manhunt 2 then which was rejected from Steam (AO rated.) though Manhunt 1 was accepted.
(Though nudity or sex are more likely to get a higher age rating than violence unless you go all out like say Punisher perhaps so this looks more M than AO.)
