i've long gotten used to scene releases that come with 1 click installations but are now seeing lots of(more like most) games that require manual file exctraction/pre-cracked games that circumvent harsher DRM instead of downright beating them(thx 3dm).
so my question is how do you properly uninstall these? do they still tie themselves to the registry which require proper uninstallation or can you just delete the game folder when you're done?
this is in regarding MGS5 but also goes for every other type of game with the same installation method.

Most games come with an installer, usually from CODEX/RLD/etc releases, but for the likes of 3DM; they're just "extract and play" titles as you rightly said. They won't add any registry entries *unless* the game itself uses them.. such as Unity titles which store their configuration details in the registry. There's no automated way of removing that stuff when you remove the game, you just shift-delete the folder and then use a registry cleaner or something to clean up remnants.

I use Sandboxie for installing games. With this setting:

It installs directly to disk, but without any reg entries or 3rd party software.

How do you play games that require registry entries? Like the aforementioned Unity titles?

You can actually run them in sandbox without problems most of the time. Which in itself is not bad idea as far as backing up saves are concerned (you'd need to only backup one folder - sandboxie itself to backup all saves) without worrying where each game/crack put its saves unless it's in the game folder.
Also I haven't played unity much, just around hour, but it worked just fine without any registry entries. At least I didn't found something that wouldn't work. What was supposed to happen if the registry entries were missing? I was using 3DM crack I believe...
Anyway, there aren't many games that require reg entries. If there are I'd personally run game under sandboxie, or alternatively run regedit under sandboxie and export respectable registry keys that are needed to run and import them to my normal registry.

At least CODEX always comes with an uninstaller in the install folder.
But i am not sure if those indeed remove the registry entries from the game itself.

I might look into it, having everything confined to a single location would sure as hell make things easier for backing up when I do reinstalls.

No, silly, not Assassin's Creed Unity Unity-engine Dex, for a recent example, stores not only game settings but also the entire game SAVE in the registry.

Yeah, if you've got an installer for the game then the uninstall takes care of removing all the files - such as reg entries.

why would you bother installing with full disk access at all, when you could have them all neatly in separate sandboxes?
install and full delete with a few clicks
