Well, although Rome never had a black emperor, they had black senators, since black-populated provinces were under their perview at some point, locals got senators to represent them.
Nobody really gave a shit about your skin colour back then, the only thing that mattered was your culture (or perceived lack thereof aka barbarians) and your social standing.
So I really don't see what's the big issue here. Macrinus was most likely Berber and not black, but it's not outside the realm of possibilities.
