With the Orcs you basically wipe out rebel supporters in the towns and finally the "hidden" rebel hideouts.
Spoiler:
Eventually you can venture up to I forgot what it was called but the town next to the pass to Nordmar and talk to Khan whom leads the invasion and you will remove Rhobar and Xardas I believe ultimately following I think it's Beliars ending by supporting Zhuben in the desert, or you could kill him too but then the game would be impossible to finish as far as I know since it's either Rhobar with Innos, Zhuben with Beliar or Xardas with Adanos in order to "finish" the game.
Oh and if you do side with the orcs you get a warning when you head to to Nordmar about how they will still attack you so that part is still the same, no point in attacking the barbarians as that will just have you kill every NPC in Nordmar in the end.
(Denying you access to the ancient forge and possible rune-weapon forging and other high-end weapons unless you loot the special chests until you get the ultimate rewards from them, I think there's 53 of them or some such, darker in color and often locked with crap rewards starting out but as you find more of them you eventually get the best weapons in the game.)
Also instead of Paladin armor you will be wearing the orcish variant of it which I think has slightly higher defense but it looks pretty much the same albeit yellow tinted and some symbols on it.
Oh and you don't really get much NPC support, larger overhaul mods are probably going to be required before that particular mechanic is fully fleshed out, currently it's a bit superficial since "winning" for one faction is just another quest to complete with no real changes other than the gold and XP reward, as far as I remember.
this is a good package if you want to try this unpolished gem again... installed it with the ton of Union mods that comes with it, runs fantastic and no longer with the typical hitching that plagued the game (yes even the last time when i played it heavily modded with latest fixes/patches).
on my NVMe M.2 SSD it's crazy how fast it loads and no stuttering at all, pretty amazing how they managed to fix that, to me the stuttering has been a deal breaker when i've tried it again...
It's far better than when it came out, and you can make it better by using a mod that makes the animations better. But i agree, they're still pretty bad...
The whole game was a giant mess on release. I was so hyped and then it was running at 20 fps even on the latest PC's.... I had to use stupid immortality cheat code because it was impossible normally. And even if You-Know-Which-Animal didn't exist.
I ran thru this a few years ago just to say I beat it and can’t say I enjoyed it. Just awful combat and checklist style open world gameplay. And the story even ended on a cliffhanger haha.
It was marvelous when it came out due to the size of the open world but the cracks show pretty soon obviously.

Didn't the expansion solve the cliffhangers? Even if it was, allegedly, one of the worst expansions of all time.
