It's smoking really but drinking does do some things

. Smoking increases your chances of stroke but if her blood pressure is fine she probably has nothing to worry about but everybody is different. The amount of hormones in the pill today are so low (but she could be on a higher dose) Its almost like a mini pill compared to what was.
- smoking also increases metabolism so estrogen is metabolised faster and can cause spotting


-if she drinks to much and pukes in the morning or whenever she just took it, that will effect the effectiveness of the pill.
- Alcohol also keeps your liver working harder and so the hormon levels are filtered slower. Estrogen levels tend to be higher in someone who drinks, but if that person stops drinking and only if that person is used to drinking regularly their body will filter the hormones quicker and cause spotting (bleeding but not period)

There is something about grapefruit juice slowing the metabolic functions of the liver and can prevent spotting.

All this should be on her prescription handout. She probably just doesn't want to drink. Your friend can even find out what pill she is on and ask the pharmacist himself.
