Well, it's cooming. Like it or not, and once again I'll be doging prank phonecalls left and right, but I also want to pull a nasty one on my girlfriend. I was thinking I'd fake blood or something and just scare her shitless, but that's a little out of touch. While I'm pondering, feel free to share your kodak moments for mutual inspiration.

I say start cutting toes, then break the knees, then the fingers on the hands, then bend the arms so that you break the elbows, then dislocate the shoulders, then rip the arms, then raise them by their hair and shake until it tears. Then move to the guts; have a knife through them, while showing the pranked one his internal organs. Finally, to maximize the effect, cut the heart and quickly show it to the pranked while still alive.
Nothing beats that.
[EDIT]Hm, hold on... If the pranked one is a male, bring a stripper or something to turn him on first. Now, cut off that cock. With the cock, you could prank another one, with say, a "finger" in the box prank.
