Sounds pretty simple to me, delete the game folder manually, use CCleaner or a similar utility and delete the program from the uninstall list.
Run regedit and delete any information about the game.
(HKey_Local_Machine/Software/RockstarGames/Grand Theft Auto IV/ )
This is optional but will get rid of the last couple of files, delete the information in the "Documents and Settings" or "User" folder about the game.
(On XP it should be - "C:\Documents and Settings\Username\My Documents\Rockstar Games\GTA IV" and "C:\Documents and Settings\Username\Application Data\Rockstar Games" )
this is why i install everything in a sandbox
don't have to worry about the OS getting screwed up
btw, save the gta main folder when you delete everything else
installing to the same folder the next time makes it go much faster when the files are already there
takes ages to install normally
