This is a common issue apparently, if you look on the official support boards. I reinstalled and completed the final parts of the game without the patch because of this bug.

Damn I can't believe they still haven't fixed the NPC's falling to their deaths or off the map. It just happened in my game with the leader of Paradise Falls.

Which version of this should I download? Is reloaded's release really of the russian starforce version? So it won't work with FOSE?
Which releases work with FOSE?

http://xbox360.gamespy.com/xbox-360/fallout-3/946136p1.html
Operation Anchorage (The first DLC, out on January 27th.) Q&A asked by the fans and compiled by Gamespy. (Shown on Bethesda's blog, Bethblog.com)
For the X360 edition but it's not like the actual content will be different on either platform.
(Aside for the PS3 users though, Microsoft ruined it for them by a contract with Bethesda for the addons to be only on LIVE, both for Windows Live or XBox LIVE though.)
EDIT: Pfft lore breaker on that last question and it's answer.

Patch strengthens Live! integration actually, there's a download button showing new updates and additions for the game when signed in now for example and it's required for the DLC's.
(Sort of I guess, they use LIVE marketplace for you to buy them but I guess it's a simple setup file with a common ESP and BSA combination, BSA for the files as it's a packed archive and ESP as it's the plugin format.)
That said Live! is integrated pretty lightly in this game, renaming the .cat file from Fallout3 to for example Fallout3_.cat works just fine and progrmas like Fallout Mod Manager and Fallout Script Extender can also disable Live!.
(FoMM mainly disables Live by running FoSE though <- Those are acronyms for the mod manager and script extender by the way.)

I don't see a version number either but the game EXE reports as being version 1.1.0.35
