Thanks for posting, good video and explanations. It looks like that with the update at least NPC encounters feel more natural than before, especially for people starting a new game.
Those encounters do feel quite ok to me -- I never saw any monsters / NPCs popping in and out around. And they sure do make the world 'feel' more alive - even more than CP2077.
Go figure, huh.

• Fixed an issue where a player could lose the ability to move after loading the save made right after the explosion of a C4 device.
• Fixed an issue where Mavka could still be present in the Kurin hub after the end of the In the heart of the Iron Forest mission.

After nearly 2 years I finally started playing (played 5-6 hours back when released and had to stop and uninstall it) after seeing all the positive feedback for this big update and that even our resident grouchy humper is saying good things about it.
Around 20 hours in, I'm finding it very enjoyable, it is atmospheric, it is immersive, it feels reasonably balanced difficulty-wise and it's looking and performing great, just don't expect the same exact feeling as the original trilogy and you should be OK.
What I do dislike is the running around like a prick between POI(s) and HUB(s).
Don't want fast travel free from / to anywhere(!), but they could at least allow you to fast travel back from POI to the main HUB in the respective sub-area...
Could save some tedious time wasted drinking and running, drinking and running, drinking and running, drinking and running, drinking and running, drinking and running, drinking and running, drinking and running...

Even with a thunderberry artefact, that one seemingly barely helps stamina regeneration.
And some areas are really badly designed with only a guide per area or worse, zero fast travel options.

Finished the DLC, gold. Pretty long as well, and I love my new gun (from a sidequest). The new mini-boss is annoying, Amok, the dream sequence a bit annoying but interesting. Also some platforming.
One of the Goat's and one of my 3 Goats, no more stuttering, looks even better now, 0 crashes (didn't have those before this patch), map is maybe the best big one in any game, just hunting for artifacts, cashes, avoiding anomalies&mutants, you can really get lost in the adventure every time and have great fun. Graphics are top notch, textures on epic, everything else on high.
Guns and gunfights the best for me, guns feel real and lethal, they can jam, breakdown, peek left and right, mod the guns.
Will replay when finished and make all the different choices.
